  "abstract": "Wearable EEG devices enable home sleep monitoring but require systematic spectral validation before their physiological outputs can serve as proxies for polysomnographic features. This study provides comprehensive spectral validation of the Zmax EEG headband against concurrent PSG using the Wearanize+ dataset. Seventy-one participants with adequate signal quality underwent simultaneous home PSG and Zmax recording. Bandpower correspondence, calibration robustness, within-subject reliability, lateralisation, and spindle detection were evaluated across all sleep stages. Zmax systematically underestimates bandpower across all frequency bands (bias -0.41 to -0.74 log units), attributable to the active Fpz reference electrode. A per-subject N2-referenced calibration eliminates this bias; N2 calibration outperformed N3 and REM alternatives (mean post-calibration r=0.601 vs 0.479 and 0.489). Post-calibration spectral correspondence was strong for alpha (N3: r=0.806) and sigma (N3: r=0.752). Within-subject reliability was excellent (split-half r>0.99). We evaluated case detection, isolation, contact tracing, reactive ring vaccination, and community vaccination (20-80% coverage). Base-case vaccine efficacy was 45% and included post-exposure protection against disease and mortality. Standard analyses treat insulin as a confounder rather than a mediator, obscuring these distinct causal channels and patient-level heterogeneity in carbohydrate response. Using meal-centered continuous glucose monitoring windows from twelve adults in the OhioT1DM cohorts, we apply causal mediation analysis to decompose the total effect of carbohydrate intake on glucose change into average direct (ADE), insulin-mediated (ACME), and total effects, by meal type and across outcome quantiles. To adjust for pre-meal confounding, we introduce a Causally-constrained Linear Autoencoder that learns low-dimensional representations improving adjustment for measured pre-meal state. Across cohorts, carbohydrate intake raises postprandial glucose chiefly through a large direct effect that insulin only partially offsets. Functional septorhinoplasty (fSRP) can improve olfaction in persistent C19OD by increasing nasal airflow to the olfactory cleft and improving olfactory epithelium (OE) stimulation. However, this implies a retained and functioning OE in C19OD patients. This study evaluates the 12-month outcomes of fSRP and investigates the histopathological correlates supporting the role of nasal airflow improvement to restore olfaction. Methods: This observational cohort study enrolled 12 patients with persistent C19OD who underwent fSRP. Olfactory function (Sniffin Sticks TDI scores) and nasal airflow were assessed at baseline, 3, 6, and 12 months.   "abstract": "Background: Interocular asymmetry of axial elongation predicts accelerated myopia progression in adults, but childhood prevalence and dynamics are uncharacterised, with no standardised metric. A scale-invariant metric is proposed and characterised in a paediatric cohort with progressive myopia. Methods: A retrospective cohort of 267 children (5-16 years; 913 follow-up intervals) with progressive myopia and routine optical biometry collected during 2015-2026 was analysed. The Relative Asymmetry Index was defined per interval as the absolute interocular axial length difference normalised to the larger eye's change; its patient-level median defined the Cumulative Relative Asymmetry Index (CRAI). An annualised Absolute Asymmetry Index (AAI) was introduced as a complementary rate metric. Results: Median CRAI was 22.7 % [interquartile range 12.8-38.8]; median AAI 0.073 mm/year. Statistically detectable asymmetry (AAI > minimal detectable change at 95 % confidence) was present in 15 % of patients; 13.9 % showed pronounced asymmetry (CRAI > 50 %; AAI 0.218 mm/year).   "abstract": "Copy number variations (CNVs) can serve as important clinical biomarkers for tumor classification and stratification. However, the utility of these CNV biomarkers for intraoperative tumor assessment within the timeframe of neurosurgical procedures has remained elusive due to the protracted duration of conventional CNV characterization methods. Here, we introduce CNVisor, a statistical framework for reliable and robust CNV detection from long-read sequencing, even under ultra-low coverage. Applied to neurosurgical tumor specimens, the proposed method enabled genome-wide CNV profiling and identified clinically relevant CNVs using roughly 60,000 reads within 20 minutes of sequencing. Integrating CNVisor with methylation-based classifiers can further reduce turnaround time and increase the accuracy of glioma subtype stratification. Together, these findings establish real-time CNV profiling using ultra-low coverage nanopore sequencing as a feasible strategy for intraoperative, genomics-informed assessment of CNS tumors.",

Current diagnostic methods using fetal biofluids provide limited insight into fetal pathophysiology. Extracellular vesicles (EVs) in amniotic fluid (AF) are a promising source of stable biomolecules that reflect real-time fetal physiology. This proof-of-concept study compared amniotic fluid EV (AF-EV) characteristics in fetuses with severe CMV infection with those of uninfected fetuses and aimed to develop hypotheses about fetal response to cCMV in utero. Methods: AF samples were collected from pregnancies with symptomatic CMV infection and gestational-age-matched uninfected controls (4 pairs, n=8 total). EVs were isolated and characterised by Western blotting, cryo-electron microscopy, and nanoparticle tracking analysis.   "abstract": "In the pathologically and clinically heterogeneous neurodegenerative disorder amyotrophic lateral sclerosis (ALS), objective biochemical predictors of survival are essential to handle complexity in clinical trials, enrich clinical decision-making and interrogate the biology of disease progression. In this longitudinal study, we performed high-depth proximity extension assay proteomics using 1,095 samples of serum (N=851) and CSF (N=244) from 426 people with ALS, with orthogonal replication in an external cohort of 349 people with ALS. Age- and sex-adjusted Cox analysis identified 57 proteins in serum, including neurofilament light chain (NEFL) and peripherin, as well as five proteins in CSF, including tropomyosin 3 (TPM3) that were associated with survival (FDR-adjusted p<0.05).
