"abstract": "Background. The SRQ-20 screens common mental distress in low- and middle-income countries, yet its equivalence across groups, especially armed-conflict exposure, is rarely tested with methods that separate true invariance from an underpowered null. We evaluated its psychometric properties and measurement equivalence in Colombian adults. Methods. In 10,865 adults from the 2015 Colombian National Mental Health Survey, we assessed dimensionality, fitted a two-parameter logistic (2PL) model, and tested equivalence across armed-conflict exposure, sex, and region using multiple-group models with purified anchoring and freely estimated group means, separating true distress differences from item bias. Item functioning was equivalence-tested against a {+/-} 0.10 band on signed expected-score differences (SIDS), with test-level differential test functioning (DTF) plus severity-graded and design-weighted sensitivity analyses. Criterion validity used design-weighted ROC against 12-month CIDI diagnoses. Results. The scale was essentially unidimensional (one-factor CFI = .945, rising to .971 with four content-redundant item pairs modelled; explained common variance = .71) and fit the 2PL well, w",

This study examined whether the Maastricht Acute Stress Test (MAST) induces changes in circulating IL-17A and how IL-17A dynamics relate to cortisol and subjective stress/pain. Forty-six healthy adults (mean age: 30.50, SD = 12.86 years; 54% female) completed the standardized MAST. Serum IL-17A and cortisol were assessed repeatedly across baseline and recovery. Stress- and pain-related ratings (VAS) and trait questionnaires (e.g., chronic stress, psychological distress, resilience) were completed. There was a significant increase over time in IL-17A and cortisol following the MAST, with an 36 % increase of IL-17A level measured at 105 min post-stressor and cortisol peaking at 10 min post-stressor. IL-17A indices (baseline and AUCi) showed no meaningful associations with cortisol, subjective stress/pain, or psychological traits, whereas cortisol response (AUCi) correlated positively with perceived acute stress and pain. Overall, the MAST produced clear IL-17A and cortisol responses that appeared dissociable in healthy individu",

VSL#3TM, a high-potency multi-strain probiotic, may provide synergistic effects through microbiota modulation and immune regulation. This preliminary real-world study aimed to evaluate the efficacy and safety of VSL#3TM combined with VDZ in patients with UC. Methods Clinical data were retrospectively collected from patients with active UC who received VSL#3TM combined with VDZ for at least 12 weeks at the Second Affiliated Hospital of Zhejiang University School of Medicine in China between January 2023 and December 2024. The primary endpoints were clinical response rates at weeks 6 and 12. Secondary endpoints included clinical remission, changes in inflammatory bowel disease questionnaire (IBDQ) scores, safety assessment. Results Using PRO2 criteria, clinical response rates were 82.4% (14/17) at 6 weeks and 100.0% (17/17) at week 12, with clinical remission in 58.8% (10/17) at week 12. By Full Mayo Score, clinical response and remission at week 12 were 68.8% (11/16) and 50.0% (8/16), respectively. These response rates appeared numerically hig",

Among working-aged adults, the composition of these 24-hour physical behaviors may vary across occupations, but the extent of these differences remains unknown. This population-based study examined how the composition of 24-hour physical behaviors differs in working-aged adults across occupational groups during work and leisure time. Methods: The study population consisted of 2,809 adults from the Northern Finland Birth Cohort 1966 46-year follow-up study, who wore a hip-worn accelerometer for 14 days. The accelerometry data was combined with self-reported diaries to determine time in bed and work hours. Four occupational groups were aggregated based on occupation type and occupational skill level: high-skill white-collar, low-skill white-collar, high-skill blue-collar and low-skill blue-collar. Time spent on physical behaviors during workdays (including working hours and leisure time separately) and non-workdays were calculated. Differences in physical behavior compositions were assessed using a compositional data analysis approach ",

Dose titration recommendations are based on clinical experience, pharmacologic rationale, and expert consensus, with limited real-world evidence, especially in settings with widespread fentanyl use, where traditional titration schedules may be insufficient to address the higher opioid tolerance among people initiating treatment. This study aims to determine the comparative effectiveness of alternative titration schedules on completed induction and time to all-cause mortality. "abstract": "Rationale: Cardiopulmonary resuscitation (CPR) aims to provide oxygen to vital organs through chest compressions and ventilations, until return of spontaneous circulation (ROSC) is achieved. A major barrier to effective oxygenation during CPR is atelectasis, which impairs gas exchange and results in hypoxemia -- a condition strongly associated with decreased rates of ROSC and worsened neurological outcomes. Positive end-expiratory pressure (PEEP) is routinely used in critical care to prevent atelectasis and improve oxygenation. During CPR, however, its use is inconsistent and subject of debate. This reflects long-standing theoretical concerns that PEEP may reduce venous return, lower cardiac output, and impair the chances of successful defibrillation. However, emerging experimental and observational data suggest that PEEP may actually improve oxygenation, cardiac output, oxygen delivery, and rates of ROSC during CPR. We hypothesize that applying PEEP during CPR improves neurologically favorable survival. Given its simplicity, negligible cost, and widespread availability, PEEP has the potential to enhance outcomes from cardiac arrest in a scalable and resource-efficient manner. Obje",

Methods: A retrospective cohort analysis of data from a cluster-randomized trial conducted in 18 community-based pediatric primary care practices in Northeastern Ohio (2017-2022). Treatment receipt was determined using Medicaid claims, with treatment gap defined as fewer teeth with treatment claims than teeth found on baseline exam with decay. Multivariable logistic regression assessed the association of treatment gap with child age, sex, race/ethnicity, caregiver education, and number and location of baseline decayed teeth. Results: Of 766 eligible children, 487 (63.6%) attended the dentist within one year. Among 155/487 (31.8%) with baseline untreated decay, 90/155 (58.1%) had a treatment gap. Odontograms visually showed that decay was concentrated on upper anterior and posterior teeth. A treatment gap was associated with a greater number of decayed posterior teeth (OR = 1.90, 95% CI: 1.60-2.30) and decayed anterior teeth (OR = 2.19, 95% CI: 1.51-3.39), both p < 0.001. Other soci",

"abstract": "Measles remains a significant global public health threat despite the availability of an effective vaccine. WHO recommends the first measles vaccine dose at 9-12 months of age, balancing maternal antibody interference and immune system maturity. However, infants younger than 9 months remain highly vulnerable, especially in regions with high transmission and low herd immunity. This study introduces an age-stratified, multi-compartmental model of measles transmission that captures infections in unvaccinated infants by explicitly modelling maternal immunity decay and early infection risks. The model's positivity is proven, and an analytical expression for the instantaneous replacement number is derived. For a non-age-stratified version, equilibrium points and their stability are analyzed. Using a bootstrap algorithm, critical parameters and age-specific transmission rates are identified by fitting the model to yearly incidence data from six measles-prevalent countries. The proposed model provides a framework for evaluating hypothetical vaccination scenarios, including different routine and supplementary dose schedules. Under the model assumptions, scenarios including MCV0 produced lar",
