MethodsIn 46 patients with Rome IV-diagnosed FD and 30 healthy controls (HC), targeted metabolomics using chromatography and mass spectrometry was conducted to quantify metabolites in blood, urine, and stool. Associations with gut microbiota and symptomatology were tested using fecal quantitative microbiota profiling and validated symptom questionnaires. Resting-state functional magnetic resonance imaging in 27 patients and 36 HC enabled analysis of functional connectivity in selected brain networks. The Global Pediatric Diarrhea Surveillance (GPDS) network is a World Health Organization (WHO)-coordinated public health surveillance network investigating the etiology of hospitalized diarrhea among children aged <5 years in LMICs. The GPDS network enrolls children hospitalized with diarrhea at 38 sentinel surveillance sites in 31 LMICs. Randomly selected stool specimens were tested by TaqMan Array Card quantitative reverse-transcription polymerase chain reaction (qPCR) for 16 pathogens associated with diarrhea. We estimated pathogen-specific attributable fractions (AFs) and incidence of diarrheal hospitalizations at the global, regional, and country levels during 3 time periods: 2017-2018, 2019-2020, and 2021-2022, with a focus on the most recent results. Two independent reviewers screened studies, extracted data, and assessed quality using the Joanna Briggs Institute Critical Appraisal Checklist for Prevalence Studies. Prevalence was pooled using a random- effects logit model. A pre-specified subgroup analysis by age band was conducted. Publication bias was assessed with Eggers test and certainty of evidence with Grading of Recommendations Assessment, Development and Evaluation (GRADE). ResultsTwenty-six studies (7,840 individuals; 14 countries) were included. "abstract": "Ambulatory blood pressure monitoring (ABPM) is recommended for confirming hypertension and assessing out-of-office blood pressure (BP). However, patient burden and device tolerability may limit broader implementation. We compared participant experience with a traditional oscillometric ABPM device and a compact cuff-integrated ABPM. The PRO-BP Study was a pilot randomized crossover study of 20 adults in New York City. Participants completed two 24-hour ABPM periods over 7 days using the SpaceLabs 90227 and SOMNOmedics ABPM Pro devices. After each period, participants rated comfort, pain, sleep interference, embarrassment, noise, skin irritation, and interference with daytime activities. Both devices achieved guideline-based recording-quality thresholds. Compared with SpaceLabs, ABPM Pro was associated with greater comfort (median 7.0 [IQR, 5.0-8.5] vs 3.5 [IQR, 2.0-6.5]; P=0.004), less pain (1.5 [0-3.5] vs 5.0 [0.5-7.0]; P=0.003), and less embarrassment (0.5 [0-3.5] vs 3.0 [0-6.0]; P=0.01). Other experience ratings did not differ significantly. Participant experience should be considered alongside recording quality when evaluating validated ambulatory BP monitoring technologies.",

Methods: From October 5-November 30, 2021, a 10-minute REDCap survey was available to students, faculty, and staff 18 years of age and older at the Pennsylvania State University (PSU). Recruitment included targeted email, social media, digital advertisements, and university newspapers. 4,231 responses were received. "abstract": "SSRIs are first-line for obsessive-compulsive disorder (OCD), yet benefit is partial, weeks-delayed, and severity-gated, and augmentation is empirical. The stable-states view is established but not drug-specific. We supply the missing coupling: a calibrated object joining serotonin pharmacokinetics, a basal-ganglia-thalamocortical circuit, and corticostriatal plasticity, in which OCD is an elevated fixed point and SSRI treatment moves this bistable state across a saddle-node fold (a tipping point). With [~]35 constants fixed, two shared parameters reproduce six SSRIs (RMS 0.83 Y-BOCS points, 84 arm-timepoints) and predict a held-out trial (0.58). A fold-free model fits the means equally well, so the mean cannot separate them; they diverge only on individual-level signatures it cannot produce -- a discrete, durable off-drug remission subgroup and critical slowing -- with qualitative support in OCD and adjacent disorders, not yet tested as bistability signatures. These, not the mean, decide who remits and relapses; the same geometry reframes memantine and antipsychotic augmentation.",

"abstract": "Aim: To evaluate the association between monocyte-to-lymphocyte ratio (MLR) and cardiovascular outcomes in chronic kidney disease (CKD). Methods: We systematically searched Medline, EMBASE, Web of Science, and Scopus from inception to May 28, 2025. We included peer-reviewed observational studies assessing MLR and cardiovascular outcomes or all-cause death in CKD. Two reviewers independently screened studies, extracted data, and assessed risk of bias using the Newcastle-Ottawa Scale. Findings were narratively synthesized and p-values pooled using Fisher's method. Statistical significance was set at p<0.05. Results: Eleven studies (n = 18,631) met our inclusion criteria. The study population ranged from non-dialysis CKD to end-stage kidney disease on dialysis, with follow-up from 1 to 24 months. Five studies (n = 16,974) examined cardiovascular death and generally reported significant associations with elevated MLR; Fisher's method indicated strong overall evidence (p<0.001). "abstract": "Alternative splicing of pre-mRNA generates variant protein forms from a given gene, accounting for both functional redundancy and diversification. A novel RNA-binding protein, Pro-rich Coiled-coil Containing Protein 2B (PRRC2B), has been shown to mediate both upstream (u)ORF-dependent and independent regulation of translation initiation, which is essential for cell cycle progression and proliferation. We identified two alternative spliced isoforms of PRRC2B mRNA in human and mouse hearts, as well as in multiple cell types: the full-length and the exon 16-excluded isoform {triangleup}E16. A congenital heart disease-associated human mutation-mimicking knock-in of the equivalent variant in the mouse genome results in the depletion of full-length Prrc2b mRNA, but not the alternative spliced truncated form {triangleup}E16. This mouse model does not cause any apparent structural or functional cardiac disorders.
