  "abstract": "Dementia is characterized by a gradual loss of cognitive abilities, often accompanied by an accelerated decline preceding diagnosis. Cognitive decline in patients with dementia may lead to complications that impact mortality. Changepoint models have been proposed to identify when cognitive decline accelerates and how it progresses. Joint models with changepoints further account for dropout due to death or dementia. However, few joint models with changepoints consider semi-competing risks (e.g., dementia and death) by distinguishing transitions between various health states. We propose a joint model that combines a random changepoint model for cognitive decline with an illness-death model for semi-competing risks. We examine the proposed model with different changepoint formulations and association structures that connect the longitudinal and survival processes. Simulations demonstrate that our proposed model effectively estimates changepoints and characterizes the influence of longitudinal measures on transitions between health states.   "abstract": "Objective: Genetic disease is common in Level IV Neonatal Intensive Care Units (NICUs), yet clinicians often struggle to identify infants who would benefit from genetic evaluation. We developed and validated NeoGx, a machine learning (ML) algorithm using electronic health record (EHR) data to predict, early in the NICU stay, which neonates will require genetic evaluation within 18 months of life, enabling high-yield testing, including rapid genome sequencing (rGS), to be directed to the right infants while most beneficial. Methods: Data were extracted from the EHRs of 14,272 Level IV NICU patients including structured data and phenotypes derived from clinical text. Patients were temporally divided into development (N=11,201), calibration (N=1,080), and validation (N=1,991) cohorts. ML models were optimized using 3-fold cross validation in the development cohort to predict genetic evaluation by 18 months, then evaluated in an independent validation cohort. Results: Using predictions accumulated over four NICU weeks, NeoGx achieved a ROC AUC of 0.849 and PR AUC of 0.771. Methods A cross-sectional survey conducted from 2022 to 2025 assessed self-reported cardiometabolic risk factors, mental health, loneliness, healthcare access and utilization, and attitudes toward health-promoting behaviors among Puerto Rican adults aged 21-35 years. We calculated prevalence estimates and used chi-square tests to examine differences by sex and age group. Results Of 2,783 participants (mean age 27.3; 72.7% female; 59.9% with >4 years of college), over 40% reported >6 days of poor mental health and 20.4% poor physical health in the past month. Most had health insurance (92.2%), yet over one-third faced financial barriers to care and lacked a primary care provider (PCP). Influenza vaccination was low (30.4%), while HPV vaccination (54.7%) and HIV testing (70.7%) were higher. This systematic review and network meta-analysis will compare the effectiveness of remimazolam, propofol, sevoflurane, desflurane, isoflurane, xenon, and ciprofol, and establish an evidence-based ranking to guide clinical practice. Methods and Analysis: We will systematically search CENTRAL, Embase, MEDLINE, Web of Science, and major trial registries without language or date restrictions. Two reviewers will independently screen studies, extract data, and assess risk of bias with the ROBUST-RCT tool. Random-effects network meta-analyses will estimate pooled risk ratio (RR) for dichotomous outcomes and mean difference (MD) or standardized mean difference (SMD) for continuous outcomes.   "abstract": "Background Isotretinoin (ISO) is a systemic retinoid widely used for the treatment of acne vulgaris, yet its potential effects on male reproductive function remain unclear with previous studies reporting inconsistent findings. Methods and finding We conducted a multi-model study combining data from the Danish Young Men Study (DYMS) cohort with ex vivo cultured human testis tissue and an in vitro Retinoic Acid Receptor alpha (RAR) reporter gene assay. In the DYMS cohort semen parameters and reproductive hormone concentrations assessed on a specific examination day were compared between ISO-users (n=322) and non-users (n=4,065). Ex vivo human testis tissue was exposed to ISO and RA for 96 hours to assess changes in hormone secretion and germ cell dynamics, while the in vitro RAR assay evaluated ISO for effects on RA-induced RAR-signaling. In the DYMS cohort, ISO use was associated with reduced FSH concentrations, higher inhibin B/FSH ratio, and elevated estradiol concentrations, along non-significant tendencies of lower sperm counts. These associations persisted even after cessation of ISO use but were not evident in men first initiating ISO use 1 year after their examination date. I",

We investigated the feasibility and complementarity of combining diffusion-weighted MRI (DW-MRI) with biophysical modelling Vascular, Extracellular, and Restricted Diffusion for Cytometry in Tumours (VERDICT) and diffusion-weighted MR spectroscopy (DW MRS) for simultaneous characterisation of glioma tumour cells and the glioma TME.   "abstract": "Noncommunicable diseases (NCDs), particularly diabetes and hypertension, are a growing cause of death in low-income countries. In Haiti, cardiovascular disease has overtaken HIV as the leading cause of adult death, and the isolated Grand'Anse Department had no dedicated chronic disease facility before this initiative. This retrospective single-center cohort study analyzed 1,298 encounters among 590 patients at a dedicated diabetes and hypertension clinic (the Centre Dr. Rene Charles) in Jeremie, Grand'Anse, from April 2024 to March 2026. Standardized assessments included serial blood pressure, fasting glucose, HbA1c, lipids, renal indices, and electrocardiography. Longitudinal change was modeled with linear mixed-effects models on calendar time, supplemented by paired first-to-last comparisons. The cohort was 74.1% female (mean age, 58.2 years), and nearly two thirds had primary-level education or none. Diagnoses were hypertension alone (52.9%), diabetes with hypertension (26.8%), and diabetes alone (20.3%). Rather than treating non-convergence as an obstacle to synthesis, we propose bias-domain triangulation, a causal-structure-aware framework that treats it as a diagnostic target. The framework distinguishes actual covariate adjustments from candidate background causal structures, adjudicates the causal roles of adjustment variables and maps core back-door pathways into distinct bias domains before quantitative synthesis, thereby reframing the question from whether estimates are heterogeneous to which bias structure changes the estimate. We apply this framework to a systematically derived literature on prenatal paracetamol exposure and offspring autism spectrum disorder or attention-deficit/hyperactivity disorder, comprising 22 studies and 33 adjusted estimates.
